Would the extended release tabs in a higher dose (200 mg.) be just as effective as 4 regular 50 mg. tabs at once? The regular tabs work well when I take 3-4 at a time, I just hate taking so many pills at once -- but it does control the pain.

I was prescribed tramadol for really bad sciatica at the beginning of May, at the peak I was taking 4 x 50mg immediate release every day. i was then moved to 100mg slow release x 2 a day. They weren’t as effective, so I went back to 4 x 50mg. I have gradually reduced the dose to 2 x 50 mg immediate release every day.
The slowed release cuts down on a lot of the unpleasant higher-dose side effects that come with instant release high-dosage tramadol. With instant release I prefer to "ramp" my dosage up. (e.g. taking 1-2 pills every 30mins or so until I reach the desired dosage) because it cuts so much of the side effects out.

I have been taking 50-100 mg. Tramadol every 4-6 hours, 2-3 days out of the week. More often than not, it takes 100 mg. to help. So I was going through the "30 day" supply in about 2 weeks. Today I asked her for more per bottle so they would actually last a month.
Tramadol immediate-release tablet: If you have severe kidney problems, your doctor will likely prescribe you 50–100 mg every 12 hours. The maximum dosage is 200 mg per day.
I have taken Tramadol for the last 2 years. I briefly weaned off last summer, but started back up around Christmas. For me, it has been a good pain medicine, plus the added benefit of depression/anxiety relief. As of right now, I only take 1-2 50 mg. tabs a day along with Celebrex and Zanaflex. I haven't had any problems.
